We specialize in treating clients suffering from addiction as they withdraw from their substance use, and transition to Residential programming. Promoting a healthy environment for addicts seeking recovery from addiction, in a luxury setting.

DUTIES: include
CASE MANAGER DUTIES: knowledge of STD, unemployment, FLMA, coordination of benefits, writing lab ordes, arranging outside appointments
· Complete all case management documentation in a timely manner
· Identifies and resolves problems in a timely manner as well as skillfully gathers and analyzes information.
· Responds appropriately to a client in crisis. Uses a variety of methods to offer short term, immediate help to individuals who have experienced an event that produces mental, physical, emotional or behavioral distress
· Prioritizes and plans work activities; uses time efficiently; and develops realistic action plans.
· Serve as a liaison between administration, patients, physicians and other healthcare providers
· Demonstrate the ability to be flexible, organized and function under stressful situations
· Act as a single point of contact for all parties involved in their client's well-being and care, with the goal being to facilitate care coordination.
· Arrange medical appointments, transport clients to meetings with other involved parties or agencies, discuss a patient's treatment plan with other professionals working at the residence, such as social workers or counselors.
· Perform administrative duties related to a client's care, such as filling out forms for social services benefits and arranging other services as needed throughout the duration of the client's residence in the facility.
· Assisting client with practical needs like finding housing, helping them find employment and arranging for continuing outpatient treatment after being discharged from the residence, if needed.
