This position provides comprehensive, compassionate social work case management services for older adults, their families, caregivers and significant others. These services include a combination of psychosocial assessments, person-centered care plan interventions, caregiver support, pro-active planning for life changes and opportunities for education via workshops, trainings, and conferences.
Essential Job Functions
This position encompasses the standard duties and requirements for any Case Management position at Avenidas, but with special emphasis on heightening our current inclusive practices to a targeted focus on the low-income and other underserved communities and their needs. Primary duties include:
· Provide screening and intake by telephone regarding client’s concerns, requests for information, caregiver support, elder care consultation or case management. This may involve providing information on area resources regarding housing, legal aide, transportation, home care and financial assistance or referral to one of many Avenidas programs, including the Rose Kleiner Adult Day Health Program, Enhanced Care Management, Door to Door transportation, in-home tech support and/or membership in the Avenidas Village.
· Conduct both brief and comprehensive inclusive, geriatric, psychosocial assessments of clients’ needs (in the office and/or during a home visit) for emotional and social support, community resources, information, guidance, intervention and client advocacy. The Case Manager will act as an advocate in locating housing, home care, legal and financial aid and all community resources, keeping files updated and current.
· Provide brief community- based case management services to link seniors and their families to needed services, provide follow up and advocacy on behalf of older adults in need of resources. Demonstrate empathy while also maintaining professional boundaries.
· Case Manager will participate in outreach and learn area resources to connect case management clients and information and assistance callers to needed resources. Cultivate community partnerships for cross-referral, awareness, and educational purposes.
· Maintain accurate records of client contacts through electronic chart documentation, charting all client contact, referrals made, and other work on behalf of clients within 48 hours of service.
· Ensure documentation is completed accurately and timely for supervisors to use for reporting requirements.
· Monitor outcomes of interventions to ensure client needs are met through client satisfaction surveys and enhanced wellness, emotional and or social outcomes such as increased socialization and decreased social isolation.
· Secure budget approval from supervisor for all related expenses for client emergency fund assistance or other program expenses.
· Attend weekly supervisory sessions for clinical and administration consultation and agency meetings as needed for guidance on all job responsibilities. Attends training programs and educational seminars as approved by supervisor.

Qualifications
Education and Experience.
· Bachelors’ degree with two years of case management experience with older adults or Masters in Social Work preferred. (Candidates with an MSW degree will be titled Social Worker and have corresponding pay).
· Relevant experience with high school diploma or equivalent will be considered.
· Bilingual in Spanish, Mandarin, Cantonese and or Vietnamese preferred but not required.
Knowledge, Abilities and Skills.
· Knowledge of and capacity to utilize community resources; ability to assess physical and social needs of frail older adults. Strong oral and written communication skills.
· Bilingual highly preferred.
· Demonstrates ability to provide assistance in a culturally sensitive and respectful manner to all persons.
· Demonstrates creative, person-centered care planning.
· Has car with minimum insurance levels and able to travel as required by position.
Physical Demands
- Frequent stand/walk, sit, use a computer; use a telephone, grasp lightly/fine manipulation.
- Occasionally reach/work above shoulders, twist/bend/stoop/squat, grasp forcefully, lift/carry/push/pull objects that weigh up to 10 pounds, write by hand, sort/file paperwork.
