The Epilepsy Agency of the Big Bend (EABB) has been a cornerstone in the epilepsy community for over 30 years. Founded in 1989 as a charitable 501(c)(3) organization, EABB is dedicated to reducing barriers to care for individuals affected by epilepsy, particularly those experiencing poverty, without health insurance. Our mission is to provide vital support and financial assistance to epilepsy patients and their loved ones, offering a lifeline to those with nowhere else to turn. Job Summary
Reporting to the Executive Director, the Senior Case Manager will play a crucial role in enhancing the delivery and quality of our case management and healthcare navigation services. This position will oversee program operations, ensure compliance with grant requirements, and support clients in accessing the care they need.
Responsibilities
- Oversee and enhance the quality of case management services, ensuring that clients receive comprehensive support.
- Provide direct case management and healthcare navigation services to clients, including assessment, planning, coordination, and advocacy.
- Develop and implement program goals, strategies, and evaluation methods in conjunction with the Executive Director.
- Manage the recruitment, training, and performance of program staff in collaboration with the Executive Director.
- Ensure ongoing staff development and high performance through regular training and performance reviews.
- Prepare and submit accurate program and administrative reports.
- Ensure compliance with all grant and contract requirements in partnership with the Executive Director.
- Collaborate with healthcare providers, community organizations, and stakeholders to improve service delivery and client outcomes.
- Identify and address barriers to care for clients, particularly those related to poverty and lack of health insurance.
- Develop and maintain partnerships with local healthcare providers, social services, and community organizations to expand resources for clients.
- Conduct regular program evaluations to identify areas for improvement and implement changes as needed.
Qualifications
- Bachelor's degree in Social Work, Public Health, Healthcare Administration, or a related field; Master’s degree preferred.
- Minimum of 5 years of experience in case management, healthcare navigation, or a related role in a non-profit or healthcare setting. Advanced education may be substituted for some of the required years of experience.
- Proven experience in a leadership role, with the ability to manage and develop high-performance teams.
- Strong organizational and project management skills.
- Excellent written and verbal communication skills, with the ability to communicate effectively with diverse stakeholders.
- Knowledge of epilepsy and related healthcare issues is highly desirable.
- Ability to work independently, with a strong sense of initiative and responsibility.
- Experience with grant management and compliance is preferred.
- Proficiency in using case management software and other relevant technology.
- Experience in crisis intervention and the ability to handle emergency situations calmly and effectively.
- Familiarity with local healthcare and social service systems.
