Job Qualifications
Education:
A BA/BS in an appropriate field, such as education, special education, psychology, sociology, or human relations,
Five years’ experience in the field of developmental disabilities, OR
Some combination of education and experience
Experience:
Experience working in the human services field as a case manager or direct service worker is preferred, although not compulsory.
Other Requirements:
- A criminal and Motor Vehicle record which meets company.
2. A valid Colorado Driver’s license.
3. Verified legal status to work in the United States.
4. Excellent computer skills specifically in Microsoft Office Suite.
Physical Demands/Working Conditions
This position requires frequent walking, sitting and occasional standing and driving; occasional balancing, stooping, bending; frequent handling objects, and reaching with hands and arms; and constant finger movements. The position requires occasional lifting, carrying, pushing or pulling objects up to 25 lbs. The position also requires ordinary talking and hearing capability; full vision capabilities; and the ability to read/comprehend, write, communicate orally, and reason and analyze on a constant basis, and to perform calculations frequently. Work is performed using a computer and standard office machinery daily, and occasional use of a vehicle.
There are inherent risks in supporting people with intellectual and developmental disabilities. May be subject to name calling or other verbal insults in addition to physical aggression including, but not limited to, hitting, kicking, spitting, slapping, punching, throwing objects, etc. MVDS provides general and specific training to best prepare staff to prevent, and/or manage these occurrences.
Position Summary
The Service Coordinator - Children’s Program is responsible for the overall coordination of services and supports to assure that individualized programs, based upon identified needs, are developed and delivered in a manner which will provide each person served with the opportunity to progress to the fullest extent possible.
Essential Functions
Service Coordination
- Assure that individualized services and supports are delivered in accordance with the provisions of the IFSP; and utilize all appropriate and available resources.
- Facilitate communication between the family, MVDS, and other resources as appropriate.
- Act as an advocate for the family, to assure their voice is heard and priorities are supported.
- Coordinate and assist in the development of the IFSP including:
- Assure that staffing are held in accordance with regulatory requirements;
- Gather and summarize assessment information;
- Identify and prioritize the individual/family program needs;
- Serve as facilitator of staffing committee;
- Recommend special staffing to make program adjustments as necessary.
- Monitor the IFSP and the child’s progress through the ongoing development and review of progress reports, consultation and technical assistance with staff and independent contractors.
- Assure that persons served and family members receive timely notification of IFSP staffing, or other events in accordance with DDD and CDE regulations.
- Act as the Family Consultant for the Family Support Services Program and develop the IFSP with the family;
- Request monetary resources from the Resource Allocations Committee based upon needs determined in the IFSP;
- Assist families in obtaining resources within the community;
- Submit receipts for payments to the families;
- Monitor usage of funds and participation in program.
- Assist qualified individuals and families with the procurement and ongoing receipt of SSI, Medicaid, and external assistance and resources as identified by the interdisciplinary team.
Programming
- Assist in the coordination of Child Find activities as necessary to assure fulfillment of community needs, including:
- Community outreach and intake
- Completion and/or review of screening instrument
- Coordination of interdisciplinary assessment process
- Submission of documentation necessary for eligibility determination.
- Serve as Mountain Valley Developmental Services representative on committees, task forces, etc., which may further the effective meeting of the needs of persons with developmental disabilities.
- Participate in the local community as the Part C Liaison. Duties within the local community may include:
- Coordinating with and supporting the Director of Child and Family Services with public relations and education about MVDS programs; especially the Early Intervention/Part C and Family Support Services Program,
- Assisting with meeting the timelines and assurances of the Part C requirements,
- Assisting with referrals as needed,
- Attending meetings and trainings as needed or requested and,
- Assisting the Director of Child and Family Services with gathering and inputting data, and assisting with community collaboration as requested.
- Attend Child Outcomes trainings as necessary; Disseminate Outcomes information; coordinate Child Outcomes activities with appropriate persons; and assist in the oversight of the program by communicating with staff, providers, families and EI Colorado, in regard to implementing and reporting Child Outcomes program.
Administration
- Establish and maintains client files to adequately meet regulatory requirements
- Enter targeted case management contact information within 48 hours of the contact.
- Provide information for entry into the CCMS/ UniCORN systems in a timely manner to Director of Child and Family Services in order to maintain current and accurate data.
Other Required Duties
Standards of Behavior/Conduct
- Respond to all requests for information or communication in a timely manner.
- Report all on-the-job injuries to immediate supervisor and Director of Human Resources within 24 hours of the occurrence and provide a detailed account of the events leading to the injury.
- Respond as a Mandatory Reporter. Report all incidents of suspected client rights violations, mistreatment, abuse, neglect or exploitation immediately according to Colorado state law and MVDS policies and procedures.
Attendance and Scheduling
- Report for work on-time and as scheduled.
- Enter daily, accurate timekeeping records in timekeeping database.
- Attend all regular staff meetings and other meetings as requested by supervisory or administrative staff members.
- Work schedule as assigned.
- Have all time-off requests pre-approved.
- Be responsible for own transportation to work.
Competencies and Certifications
- Complete all mandatory new hire requirements and training as identified by the State of Colorado and Mountain Valley’s training requirements within 90 days of hire and maintain courses and certifications as required.
- Maintain skills with computers, take advantage of new technology, learn new tools quickly, use technology to enhance job performance and increase productivity. Maintain current understanding of technical processes/equipment.
Adherence to policies and procedures
- Adhere to MVDS policies and procedures outlined in the policy manual, Employee Handbook, and other employee policies.
- Complete other duties and responsibilities as assigned to ensure effective functioning of Mountain Valley’s programming.
