The Mosholu Montefiore Community Center (MMCC) is a vital community resource that changes the course of lives of Bronx and Manhattan residents to a degree not otherwise possible. Major Responsibilities:
- Counseling and Therapy: Provide individual and group therapy to address emotional and behavioral challenges, anxiety, depression, self-esteem, and coping skills. Tailor interventions to the unique needs of each client.
- Advocacy: Advocate for clients within educational institutions, vocational programs, and community agencies to ensure they receive the appropriate accommodations and services outlined in their IEPs.
- Skill Development: Assist clients in developing essential life skills, including financial management, communication, problem-solving, and self-advocacy, to promote independence.
- Family Support: Provide guidance and support to clients' families, helping them understand the transition process and their roles in supporting their young adults.
- Continuing Education: Stay informed about the latest research, therapies, and strategies related to youth with IEPs to ensure the provision of evidence-based care.
- Conducting interviews with service users and their families to assess and review their situation, complete ISS for each participant in the program. Set goals and timescales; review and revise as appropriate and document progress in achieving goals and outcomes.
- Case notes, retention and engagement, intake and assessments, referral documentation and follow up.
- Offering information and support to service users and their families.
- Crisis interventions services and follow-up as needed.
- Recommending and sometimes making decisions about the best course of action for a particular service user.
- Liaising with, and making referrals to, other agencies.
- Participating in multidisciplinary teams and meetings regarding, for example, child protection or mental health.
- Maintaining accurate records and preparing reports.
- Conduct training for staff and employers on best practices in working with special needs populations and removing barriers to education and employment.
- Participating in all mandated professional development training and meetings as relevant to position.
Requirements:
● LMSW or LCSW
● Minimum three years of experience working with opportunity youth ages 18-24 who are participating in a workforce development program.
● Must be able to effectively work with young adults in both individual and group settings including those who may have special needs (learning, social and physical); school non-attenders; and deal with other issues such as drug/alcohol abuse, homelessness, mental health conditions, etc. The goal is to support clients in becoming productive members of the workforce.
● Must understand utilization of community resources.
● Must possess excellent verbal and written communication skills.
● Must provide quality internal and external community service; demonstrate good judgement in dealing with issues.
● Must have the ability to work independently with attention to task details and meeting deadlines.
● Leadership competencies for program stakeholders
● Persistent/resilient and able to multi-task
● Be Team oriented.
● Proficient in basic use of a computer to including Microsoft Office, Word, Excel, PowerPoint, and Google
● Schedule flexibility including occasional weekends when needed.

Schedule: Day Shift, weekdays and weekends as needed (up to 21 hours per week).
